The origin of evidence-based practice can be traced back to Florence Nightingale in the 1850s during the period of Crimean War. It is a method of providing best patient care through the use of available evidence that provides the best guide to nursing care. The use of evidence-based practice is gaining popularity across all health care centers with measures being set up that promote the use of the practice by all nurses. Nursing care delivery models that explain the way task assignments, responsibilities and authority are to be handled exist and some of them further support and promote the use of evidence-based practice.
Total patient care is a type of nursing care delivery model that allows for the use of evidence based practice. Nurses are expected to plan, organize and carry out all duties involving care of the patient. The nurses provide all the health care duties to a set group of patients during a particular shift. The model gives the nurse full responsibility of care thus nurses are free to do what they deem right. The nurses are free to make decisions regarding care for the patients and are in charge of the overall care of the assigned patients. The nurses know all the needs of their patients since they perform all the tasks and not just specific tasks. The nurses, therefore, can refer to evidence and researches related to the cases of their patients and use the information to perform their responsibilities (Cherry & Jacob, 2014).
Primary nursing also supports evidence-based practice. The model gives the nurse full autonomy in providing health care for a patient. The nurse is fully responsible for a patient’s health care from the point of admission through to discharge. The nurse is fully responsible for organizing and directing the plan of care for the patient. The nurse therefore has full independence in deciding how the patient will be cared for. The nurse is; therefore, free to employ the use of evidence-based practice in designing and formulating the patient’s plan of care (Cherry & Jacob, 2014).
Clinical pathways model also provides for the use of evidence-based practice in nursing. The model involves the use of clinical management plans that indicate patient care activities and outcomes for a particular health condition. It provides specific guidance on methods of care for patients depending on the health condition and also specifies the expected outcomes. The methods of care used by the nurses, therefore, become standardized for patients with similar diagnosis. Clinical pathways model basically incorporates the use of evidence-based practice in nursing. It involves the use of verified information on the steps to take in the care of a patient with a specific type of diagnosis. Decisions made by the nurse on the care of the patient are, therefore, based on researched evidence and information (Cherry & Jacob, 2014).
The nursing process involves five steps used by nurses to ensure the quality of patient care. The process of evidence-based practice can be said to have commenced the moment the nurse decides to take up the nursing process. The nursing process is part of evidence data that has been shown to work when followed by practitioners in nursing. The first step of the nursing process is the assessment phase that involves gathering information about the patient’s general status. Information gathering can be done in different in ways the most common being conducting a patient interview, carrying out physical examinations and reviewing the patient’s health history (Orlando, 2014). The assessment phase is followed by a diagnosing phase that involves the nurse providing a diagnosis on the patient’s health problem. The nurse also includes any possibilities of further problems occurring.
